Community Health Center

Adult medicine, adolescent medicine, dental care, HIV services (including primary care, case management, counseling and testing, and support services), pediatric care, reproductive health care, prenatal and postpartum care, immunizations, school physicals, dental care, social services, and a pregnancy prevention program known as Teens P.A.C.T.

Reception Area/General Impressions of Clinic:This space was described as clean, friendly and professional. At survey time, there were no patients waiting and our volunteers were immediately greeted in a calm and polite manner. Patients must be buzzed into the waiting room. Overall, this center received very high praise from our volunteers.

This clinic will see anyone who walks through its door, regardless of age, income level, incarceration history, or immigration status. Parents may bring their children with them to appointments, though there is no childcare.

Parental consent for minors:Required for physicals; Confidential STD screening/reproductive health services for teens.

Staff has received training in working with LGBTQ populations, and if so desired, will refer queer/trans folks to program in S. Bronx where they may be seen by trans healers (including MDs) every Wednesday (Morris Heights Health Center).

Sundays and Holidays: Closed

Appointments are encouraged, walk-ins are seen as possible from 9am-4pm, always seen immediately in case of emergencies.

Physicals: $40

Family Planning, Gynecological and STD testing (including Plan B and condoms): Free

General Medicine: Sliding Scale ($0-200) determined by income as stated by patient (honor system)

All major insurance plans and Medicaid are accepted

Referrals
In case of complications or surgeries, will refer out to the main center in the network, where patients may often be seen on the same-day.
